Job Summary

The Occupational Therapist is a vital member of our interdisciplinary team, responsible for providing comprehensive occupational therapy services to our participants. This role requires a strong commitment to delivering high-quality care and a passion for working with seniors and their families.

Key Responsibilities
  • Conduct initial, semi-annual, and unscheduled assessments to identify participants' needs and develop personalized plans of care.
  • Collaborate with the interdisciplinary team to implement Quality Improvement activities and ensure seamless care transitions.
  • Perform intake screenings, develop treatment plans, and provide therapy services to participants in various settings, including their homes, hospitals, and assisted living facilities.
  • Develop and instruct staff and family members on therapy programs, ensuring continuity of care.
  • Delegate oversight of OT interventions and activities to restorative aides, as determined by the IDT plan of care.
  • Assess durable medical equipment needs, receive team approval, and order equipment as necessary.
  • Communicate with staff, participants, and families regarding services available through occupational therapy.
  • Train and supervise restorative aides on site, reporting to the supervisor as required.
  • Provide training on body mechanics and ensure adherence to departmental policies.
  • Review and revise restorative exercises as indicated, and consult with the activities department regarding activity adaptations.
  • Develop special programs according to participant needs and complete documentation/reports as required.
  • Adjust schedules as needed to meet the needs of our participants and maintain confidentiality of participant information.
Requirements
  • Minimum of one year of documented experience working with a frail or elderly population.
  • Current Occupational Therapist license in the State of California.
  • CPR with First-Aid certification.
  • Medical clearance for communicable diseases and up-to-date immunizations before engaging in direct participant contact.
