Job Summary:

As a Healthcare Admissions Coordinator, you will leverage your clinical expertise in a dynamic business setting. Successful candidates will engage with the community to promote the services offered by our facility and foster strong relationships with families, caregivers, medical professionals, and the communities we serve. By cultivating long-term connections, the Healthcare Admissions Coordinator will assist in identifying and evaluating suitable patients for potential admission to our rehabilitation services.

Key Responsibilities:

  • Review patient medical records and possess a comprehensive understanding of medications, diagnostic results, therapy assessments, and pre-existing health conditions.
  • Communicate effectively and professionally with patients and their families while collecting additional clinical information and medical history.
  • Conduct thorough assessments to identify patients who may be eligible for admission into our healthcare system.
  • Complete detailed Pre-Admission Screens in accordance with facility protocols and insurance requirements.
  • Research and analyze comparable programs and alternative care levels in the area.
  • Provide updates on patient status to physicians, insurers, case managers, social workers, and other relevant stakeholders.
  • Collaborate with the Program Director, Medical Director, and Nurse Manager to develop educational and community outreach strategies.
  • Maintain open lines of communication with patients, families, healthcare providers, employers, and team members.
  • Participate in team meetings alongside the Medical Director, Program Director, Nurse Manager, Therapy Coordinator, and others as needed.
  • Serve as a resource for team members, offering in-service training on rehabilitation criteria, case management, coding, and more.
  • Contribute to the development of clinical programs.
  • Educate specialized physicians on quality standards, program details, accreditations, and regulations related to inpatient rehabilitation.

Qualifications:

  • Current RN or LPN license in the state of practice is required.
  • Associate degree in nursing is required; Bachelor's degree is strongly preferred.
  • Valid state driver's license and reliable transportation are mandatory.
  • Proven experience in a healthcare environment, with strong interpersonal skills and familiarity working with physicians, nursing leadership, discharge planners, case managers, and social workers.
  • Clinical experience is essential.
