Position Overview:
The Nursing Admissions Coordinator will be responsible for initiating the comprehensive care plan with the approval of the attending physician and medical director, while ensuring the admission process aligns with facility protocols.
This role involves clearly communicating the services offered to patients and their families, as well as addressing all initial patient needs. Collaboration with the Inter-Disciplinary team is essential to discuss the initial care plan, patient status, and other critical information. Strong organizational and interpersonal abilities, along with exceptional written and verbal communication skills, are essential for success in this position.Qualifications:
Minimum Education:
ADN/BSN
Minimum Work Experience:
At least 1 year of nursing experience, preferably in a skilled nursing environment.
Previous experience in an Admissions Nurse role within skilled nursing, subacute rehabilitation, or other dynamic healthcare settings is required.
Familiarity with Point Click Care (PCC) is preferred.An equivalent combination of education and experience that provides the necessary knowledge, skills, and abilities will also be considered.
A polished and professional appearance is a must.Required Licenses/Certifications:
BLS/ACLS
Current and valid RN license in the Commonwealth of Pennsylvania.
