Care Manager

We are seeking a skilled Care Manager to join our Health Homes Serving Children program at Greater Mental Health of New York. As a Care Manager, you will play a vital role in coordinating community-based care management services for children and adolescents with special needs.
Key Responsibilities- Provide outreach and engagement to children and families to assess their service needs.
- Develop and implement individualized plans of care in collaboration with families and their care teams.
- Facilitate the creation of a support network for each child and family, and convene regular meetings to monitor progress towards jointly developed goals.
- Provide crisis intervention and stabilization services as needed, and maintain ongoing communication among the child, family, and community providers.
- Document client progress and maintain a permanent record of client activity according to established methods and procedures.
- Practice with a person-centered, trauma-informed philosophy in alignment with the values of Greater Mental Health.
- Bachelor's Degree with two years of relevant experience, or licensed as a Registered Nurse with two years of relevant experience, or a Master's Degree with one year of relevant experience.
- Basic computer knowledge is required, and proficiency in using an Electronic Health Record system is necessary.
- Reliability and the ability to work independently, with a strong understanding of the importance of maintaining confidentiality.
- Possession and maintenance of a valid Driver's License with a satisfactory driving record according to our insurance carrier's standards.
- Must maintain current NYS Defensive Driving certification within 30 days of hire.
- Must maintain pre-employment clearances by applicable regulatory clearances as required.
- Legal eligibility to work in the United States.
- Salary Range: $44K-$50K, based on departmental budget, qualifications, experience, education, and licenses.
- Flexible Work Arrangements: Subject to business needs, including hybrid options.
- Paid Time Off: 25 days of PTO, 12 sick days, and 11 company-paid holidays per year (prorated for part-time staff).
