Position Overview:
As a Clinical Care Coordinator, you will serve as an essential member of the healthcare team, focusing on the management of patients with complex health conditions.
This role is designed for an on-site registered nurse who will engage with patients within the healthcare system, particularly those with chronic illnesses such as heart failure and chronic obstructive pulmonary disease.
Qualifications:
A Bachelor of Science in Nursing (BSN) is required, along with a valid New York State Registered Professional Nurse license. Associate Degree holders may be considered if they meet established performance criteria.
Prior experience in care management is essential, with a background in one of the following areas: disease management, home health services, hospital settings, or physician practices. Certification in Case Management (CCM) is preferred.
Experience Requirements:
A minimum of 5 years of nursing experience in care management is required. Familiarity with electronic medical records (EMR) is necessary, along with previous experience in leading projects. A strong understanding of community resources and insurance plans is also important.
Exceptional verbal and written communication skills are crucial, as well as outstanding customer service abilities. The candidate should be capable of working autonomously, prioritizing tasks effectively to coordinate care plans. Proven leadership skills and the ability to collaborate within a team environment are essential. The ideal candidate will be highly organized, detail-oriented, and adept at managing multiple responsibilities. Proficiency in behavioral and educational strategies, including motivational interviewing and self-management support, is expected.
Recognizing the impact of cultural, social, economic, and behavioral factors on healthcare access and delivery is vital. The candidate should be prepared to address the diverse needs of patients stemming from these differences.
Compensation:
The pay rate is competitive, starting at $44.95 per hour, and is determined based on experience, skills, and educational background.
Exempt positions under the Fair Labor Standards Act (FLSA) will receive compensation within the stated hourly rate range.
