Position Overview:

The Clinical Nurse Specialist will deliver primary and urgent care services to adult patients, adhering to certification standards, within a designated Medical Respite Program.

Key Responsibilities:
  • Lead and manage clinical and operational processes within the Medical Respite.
  • Assist medical facilities and referral sources in evaluating and accepting patient referrals.
  • Provide education and assessment for patients seeking access to Medication-Assisted Treatment (MAT) programs.
  • Implement medication management in accordance with established medical respite protocols.
  • Ensure compliance with infection control guidelines during patient care.
  • Assess and treat patients following clinical best practices for post-acute care.
  • Collaborate with case managers to connect clients with necessary healthcare and social services.
  • Deliver patient-centered health services that include self-management strategies.
  • Conduct ongoing management and education for patients throughout their treatment phases.
  • Maintain a supportive and empathetic approach in all patient interactions.
  • Utilize evidence-based practices tailored to individual patient needs.
  • Administer medications and manage both acute and chronic health conditions following established protocols.
  • Document significant patient issues and communicate them to the Nurse Manager.
  • Provide health education and advocacy services to patients and their families.
  • Participate in on-call support as required.
  • Foster positive relationships with community organizations and funding sources.
  • Ensure accurate and timely documentation of patient care.
  • Engage with patients approximately 70% of the time.
  • Communicate effectively and adhere to departmental policies regarding patient care documentation.
  • Perform additional duties as assigned by the Supervisor.
  • Act as a mandated reporter in accordance with state regulations.
Supervisory Duties:
  • Assist in scheduling and supervising Licensed Practical Nurses (LPNs) and other staff.
  • Oversee the workflow within the medical respite.
  • Coordinate patient flow from intake to discharge.
  • Maintain daily communication with the Nurse Manager to ensure quality patient care.
  • Facilitate external audits and site visits from regulatory bodies.
  • Participate in weekly meetings with the interdisciplinary team to review patient care progress.
Compliance Responsibilities:
  • Adhere to compliance standards as outlined in the organization's Code of Conduct.
  • Stay informed of relevant federal, state, and organizational regulations.
  • Comply with HIPAA and confidentiality policies.
  • Follow standards set by regulatory agencies.
  • Maintain knowledge of infection control guidelines.
Required Skills:
  • Ability to engage with resistant patients effectively.
  • Comprehensive understanding of healthcare needs in diverse urban populations.
  • Proficient in multitasking and managing patient communications.
  • Strong interpersonal skills and professionalism.
  • Familiarity with Electronic Medical Records is preferred.
  • Proficient in Microsoft Office Suite.
  • Ability to prepare detailed reports and manage information efficiently.
  • Excellent verbal and written communication skills.
  • Capacity to prioritize tasks and adapt to changing program needs.
  • Demonstrated cultural sensitivity in patient interactions.
Experience and Education:
  • Minimum of three years of nursing experience in primary or community health.
  • Bachelor's degree in Nursing from an accredited institution.
  • Valid Registered Nurse license in the state.
Compensation:

Salary will be commensurate with education and experience.
