Job Title: HICM+ Case Manager

The Institute for Human Services, Inc. is seeking a highly skilled and compassionate HICM+ Case Manager to join our team. As a key member of our homeless services team, you will work closely with consumers, treatment teams, and community partners to provide intensive case management services to individuals experiencing homelessness and severe mental illness.

Job Summary:

The HICM+ Case Manager will manage a caseload of consumers and provide individualized support to help them achieve their recovery goals. This includes developing and implementing Master Individualized Recovery Plans (MIRPs), conducting behavioral health assessments, and providing crisis intervention services. The successful candidate will have excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and build strong relationships with community partners.

Key Responsibilities:
  1. Manage a caseload of consumers and provide individualized support to help them achieve their recovery goals.
  2. Develop and implement Master Individualized Recovery Plans (MIRPs) in collaboration with treatment teams and consumers.
  3. Conduct behavioral health assessments and provide crisis intervention services as needed.
  4. Work closely with community partners to provide comprehensive services and support to consumers.
  5. Document all interactions and services provided to consumers in a timely and accurate manner.
Requirements:
  1. Minimum of 1 1/2 years of experience working with individuals experiencing homelessness and severe mental illness.
  2. Strong knowledge of community resources and services available to homeless populations.
  3. Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  4. Ability to work independently and as part of a team.
  5. Valid driver's license and access to a vehicle.
Preferred Qualifications:
  1. Master's degree in social work, psychology, or a related field.
  2. Experience working with individuals with co-occurring disorders.
  3. Knowledge of crisis intervention and de-escalation techniques.
