Key Responsibilities
  • Conduct thorough assessments of youth and family needs, identifying areas for support and development.
  • Develop and implement individualized service plans, working collaboratively with youth, families, and other service providers.
  • Provide transportation and support to youth, facilitating access to activities and appointments.
  • Establish and maintain relationships with schools, community groups, and other stakeholders to identify youth in need and promote program services.
  • Maintain accurate and timely documentation, ensuring compliance with program requirements and regulations.
Requirements
  • Bachelor's degree in a related field, such as Social Work, Psychology, or Sociology.
  • At least one year of experience working with youth, with a strong preference for previous work with families.
  • Exceptional verbal and written communication skills, with proficiency in navigating electronic health record systems.
  • Access to a reliable vehicle for transporting youth, with a valid driver's license, current auto insurance, and regular safety checks.
