  • Facilitate patient-centered care by recognizing the importance of social determinants of health and addressing barriers to effective coping.

  • Utilize expertise in care coordination and management to support patients through complex healthcare journeys.

Essential Qualifications:

  • Bachelor's Degree in Nursing or a related field.
  • Current California Registered Nurse License upon hire.
  • A minimum of 2 years of experience in a healthcare setting, preferably in Acute, Ambulatory, or Post-Acute care.
  • Experience in Acute Care Management or successful completion of the Transitions in Practice (TIP) program for Care Managers.
