Position Overview: The Supportive Care Specialist plays a vital role in delivering a variety of assistance services to individuals within their homes and community environments. Our services encompass diverse programs, including elder support (companionship, homemaking, personal care), mental health recovery assistance, and in-home support for those with developmental disabilities or acquired brain injuries.
Key Responsibilities:
- Engage in meaningful conversations with individuals, offering support, companionship, and essential resources for daily living.
- Establish and nurture a constructive and supportive relationship with participants through effective communication.
- Guide and assist individuals in achieving their recovery plan objectives.
- Maintain ongoing communication with immediate supervisors and agency representatives through daily progress documentation.
- Utilize personal vehicle or public transport to accompany clients to medical appointments or social activities as outlined in their recovery plans.
- Perform light housekeeping duties as required.
- Ability to lift items weighing up to 15 pounds.
Preferred Qualifications:
- Experience in healthcare providing direct support or a strong background in customer service.
- Dependable and responsible individuals with a genuine passion for assisting others.
- Access to reliable transportation, a valid driver's license, and current automobile insurance.
