The Licensed Practical Nurse (LPN) plays a vital role in our primary care team, working under the direction of a primary care provider to provide general clinical nursing support. This position requires a strong foundation in patient care standards and practices, acquired through graduation from an accredited LPN program or documentation of nursing education and training through an accredited Registered Nurse program that meets state requirements to take LPN boards.
Key Responsibilities- Assess, plan, implement, and evaluate nursing care required for patients
- Develop and revise patient care plans in collaboration with the healthcare team
- Provide general clinical nursing support to patients and families
- Communicate effectively with patients, families, and healthcare team members
- Graduation from an accredited LPN program or documentation of nursing education and training through an accredited Registered Nurse program that meets state requirements to take LPN boards
- Minimum of 1 year of experience working with patient care, with demonstrated proficiency in verbal, non-verbal, and written communication skills
- Current Licensed Practical Nurse license awarded by the applicable State
- Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire
