Ballard Center is seeking a skilled LPN Unit Manager to join our team. As a key member of our nursing staff, you will be responsible for providing high-quality care to our residents and ensuring the smooth operation of our unit.

Key Responsibilities:
  • Manage the unit and ensure that residents receive the highest quality care.
  • Oversee the care of residents and ensure that tasks are completed as indicated.
  • Complete initial, comprehensive, and ongoing resident care records to determine medical needs.
  • Assist with admissions, discharges, and physician orders.
  • Write, initiate, and revise plans of care as needed.
  • Teach and train unit staff in quality-of-care initiatives and compliance with policies, procedures, and initiatives.
  • Complete, maintain, and submit accurate clinical notes regarding condition and care given.
  • Promote a culture of safety to ensure a healthy practice and living environment.
Requirements:
  • LPN with current licensure to practice in the state.
  • Comprehensive knowledge of applicable nursing principles and practices to ensure quality-of-care standards for residents.
  • Strong communication and organizational skills.
  • Self-directed and able to work with minimal supervision.
Preferred Qualifications:
  • Experience in Skilled Nursing Facility (SNF) environment.
  • Nursing experience providing direct care to multiple residents.
  • Behavioral care / memory care experience.
  • Experience with Point Click Care (PCC).
