Job Summary:

As a Home Health Scheduler at Penn Highlands Healthcare, you will play a vital role in coordinating visiting disciplines to patients, including nursing, physical therapy, occupational therapy, speech language pathology, medical social services, dietician, and home health aides. Your primary responsibility will be to coordinate schedules between multiple providers and disciplines to ensure optimal patient care, efficient utilization of services, and compliance with physician orders.

Key Responsibilities:

  • Coordinate schedules between multiple providers and disciplines
  • Ensure optimal patient care and efficient utilization of services
  • Comply with physician orders
  • Interact effectively with individuals at all levels of the organization
  • Have strong interpersonal skills

Requirements:

  • High school diploma or equivalent
  • Prior home health experience, referral intake, or relative work experience in the health care industry
  • Ability to pass PA Child Abuse History Clearance
  • Ability to pass Criminal Background Check
  • Ability to meet physical requirements of position
  • Be a team player
