Position Overview

Springwell is excited to introduce a new initiative aimed at facilitating the transition of individuals from Long-Term Care facilities to community living. We are seeking a dedicated Liaison to support this vital process.

ROLE RESPONSIBILITIES:

The Community Transitions Program Liaison will:

  • Engage with residents of skilled nursing facilities to promote awareness of our services and programs.
  • Identify suitable candidates for the program based on their interest in moving to a community living arrangement.
  • Conduct person-centered planning and needs assessments through collaborative interdisciplinary team meetings.
  • Serve as the primary contact for residents, families, nursing facility staff, and other stakeholders involved in the transition process.
  • Complete referrals to relevant programs and ensure follow-up to facilitate effective community discharges.
  • Coordinate with various state programs and teams to support individuals in their transition, including assessments for community waiver programs.

QUALIFICATIONS REQUIRED:

  • A Bachelor’s Degree or higher is preferred.
  • Applicants without a Bachelor’s Degree may be considered if they possess:
    • An Associate's Degree along with a minimum of 3 years of experience in elder care or case management; OR
    • At least 50% of the coursework required for a Bachelor’s Degree, with a commitment to complete the remaining coursework within three years.
  • Strong understanding of long-term care, case management, discharge planning, and community resources to aid in the transition process.
  • Excellent customer service skills and experience working with diverse populations.
  • A reliable vehicle, valid driver’s license, and a clean driving record are essential.
  • Proficiency in Microsoft Office Suite, including Word, Excel, and Outlook.
