Home Health Physical Therapist

North Mississippi Health Services is seeking a skilled Home Health Physical Therapist to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and services to patients in their homes.
Key Responsibilities- Comprehensive Patient Assessment: Conduct thorough assessments of patients to identify their needs and develop individualized plans of care.
- Skilled Care Delivery: Provide skilled physical therapy services to patients in their homes, including exercises, stretches, and other interventions to promote mobility and function.
- Communication and Coordination: Communicate effectively with patients, caregivers, and other healthcare team members to ensure seamless care coordination and patient outcomes.
- Documentation and Record-Keeping: Maintain accurate and up-to-date patient records, including progress notes, treatment plans, and other relevant documentation.
- Education and Training: Participate in education and training programs to enhance your skills and knowledge, and to stay current with best practices in home health care.
- Education: Graduate of an accredited school of physical therapy or a related field.
- Experience: Minimum of two years of experience as a physical therapist in a home health or related setting.
- Licensure: Current licensure as a physical therapist in the state of Mississippi.
- Certifications: Current BLS CPR certification.
