Job Title: Home Health Physical Therapist

North Mississippi Health Services is seeking a skilled Home Health Physical Therapist to join our team in Verona, Mississippi.

Job Summary:

We are looking for a highly motivated and experienced Physical Therapist to provide high-quality patient care in the home setting. The ideal candidate will have a strong background in physical therapy, excellent communication skills, and a passion for delivering exceptional patient care.

Responsibilities:
  • Provide physical therapy services to patients in their homes, including assessments, goal development, and treatment planning.
  • Collaborate with the healthcare team to ensure comprehensive patient care and coordinate services as needed.
  • Document patient progress and maintain accurate records in accordance with agency guidelines and policies.
  • Participate in education and training programs to enhance clinical skills and stay up-to-date on best practices.
  • Contribute to the development and implementation of educational materials and programs for patients, families, and caregivers.
Requirements:
  • Graduate of an accredited school of physical therapy required; Bachelor's degree in physical therapy preferred.
  • Current licensure as a Physical Therapist in the state of Mississippi required.
  • A minimum of two (2) years of experience in home health physical therapy or a related field preferred.
  • Proficient computer skills and system connectivity skills preferred.
