We are seeking a skilled Nurse Care Specialist to join our team. As a Licensed Practical Nurse (LPN), you will provide nursing care under the direction of an RN, physician, or other authorized healthcare provider. Your primary responsibilities will include therapeutic nursing interventions, medication administration, documentation using the EPIC system, and collaborative teamwork with interdisciplinary staff.

Key Responsibilities

  • Ensure quality nursing care is rendered to patients in accordance with the New York Nurse Practice Act, National Standards of Practice, and Nursing and Clinical Service standards of care and practice.">
  • Utilize the Nursing Process in providing direct care of patients and families.">
  • Collaborate with registered nurses, medical social work, care managers, nutrition, Rehab services, spiritual care, and pharmacy staff to deliver comprehensive patient care.">

Educational Requirements and Experience

">

To be successful in this role, you must have graduation from an accredited school for Licensed Practical Nursing and current licensure, or eligibility for licensure, in the State of New York. Additionally, you should maintain current BLS/CPR certification and participate in continuing education and updates related to specific areas of expertise.

Work Environment and Hazards

">

You will work in a clinical setting, with exposure to Class I hazards. The job requires medium physical demands, including lifting 25 pounds frequently and occasionally lifting up to 50 pounds. You must be able to stand, walk, sit, bend, twist, squat, and reach throughout your shift.

Compensation Package

This position offers a competitive salary range of $24.00 - $30.95 per hour, based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. Pay may vary within the range based on location.
