Care Manager RN Job Summary

Swedish Health Services is seeking a skilled Care Manager RN to join our team at Swedish Cherry Hill. As a Care Manager RN, you will play a vital role in coordinating the care and service of selected patient populations through the acute care episode, across the continuum.

Key Responsibilities:

  • Collaborate with inter-disciplinary teams to improve patient care through effective utilization and monitoring of healthcare resources.
  • Assume a leadership role to achieve desired clinical, financial, and resource outcomes.
  • Work collaboratively with patients, nurses, social workers, physicians, other practitioners, caregivers, and community resources and agencies.
  • Contribute to the development of a goal-directed, age-appropriate plan of care through an interdisciplinary team process.
  • Communicate timely, relevant, and accurate information to all parties involved with a patient's care.
  • Facilitate the progression of care by advancing the care plan to achieve desired outcomes.
  • Develop and document a discharge plan through collaboration with the interdisciplinary team.

Requirements:

  • Bachelor's Degree in Nursing or completion of an accredited Nursing program with a Bachelors degree or higher in a health care field.
  • 3 years Registered nursing experience in an acute care hospital.
  • 1 year Hospital case management experience.
