Job Title: Wellness Nurse

Job Summary:

We are seeking a dedicated and experienced Wellness Nurse to join our team at Baylor St Luke's Medical Group. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and promoting healthy lifestyles.

Responsibilities:

  • Conduct comprehensive Annual Wellness Visits for eligible patients, focusing on preventive care and health education.
  • Collaborate with office staff to coordinate AWV appointments and ensure seamless patient care.
  • Review and update patient medical history, vital signs, and medication lists to ensure accurate and up-to-date information.
  • Collect health risk assessments and conduct cognitive impairment screenings to identify potential health concerns.
  • Assist with Advance Care Planning and annual preventative screenings to promote patient well-being.
  • Maintain knowledge of policies, procedures, and office equipment to provide efficient and effective care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in professional development and maintain licensure and certifications to stay current with industry standards.
  • Perform other assigned duties as needed to support the team and achieve departmental goals.

Qualifications:

  • Graduate of an accredited nursing program with a minimum of 5 years of relevant experience in healthcare.
  • Licensed Licensed Vocational Nurse (LVN) in the state of Texas, with BLS certification required.
  • Prior experience in Homecare, Case Management, Care Management, or Long Term Care Management is highly desirable.
  • Excellent communication, interpersonal, and basic computer skills are essential for success in this role.
