Job Description:

We are seeking a skilled Senior Rehabilitation Therapist to join our team. As a member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to provide comprehensive rehabilitation services.

Responsibilities:

  • Conduct thorough assessments of patients' physical, emotional, and cognitive abilities
  • Develop and implement individualized treatment plans that address patients' needs and goals
  • Provide hands-on therapy services, including occupational, physical, and speech therapies
  • Collaborate with other healthcare professionals to ensure seamless care transitions
  • Document patient progress and outcomes accurately and efficiently

Required Skills and Qualifications:

  • Active license as an Occupational Therapist in the state of Texas
  • Graduate degree or higher in Occupational Therapy
  • At least 2 years of experience in home health or rehabilitation settings
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
