Job Summary

We are seeking a skilled Nursing Intake Screener to join our Admissions Team at St. Camillus Residential Health. As a key member of our team, you will be responsible for screening patient referrals from hospitals, skilled nursing facilities, and other healthcare providers to determine eligibility for admission into our programs.

Key Responsibilities

  • Conduct thorough reviews of medical records to assess patient eligibility for admission
  • Collaborate with hospital care coordination teams to expedite the admission decision-making process
  • Represent St. Camillus inpatient programs and build strong relationships with healthcare providers
  • Ensure timely and accurate transfer of medical record information

Requirements

  • Current NYS RN license or Current NYS LPN License required
  • Valid CPR AED/BLS Certificate
  • 1-2 years full-time nursing experience, preferably acute care or rehab setting
  • Long-term care and/or hospital discharge planning experience preferred
  • Strong clinical assessment skills and working knowledge of medical charts and terminology
  • Some knowledge of third-party payer reimbursement guidelines
  • Strong relationship/customer service skills
  • Valid NYS driver's license and a reliable vehicle with insurance
