Patient Care Coordinator

The Yale-New Haven Health team is seeking a dedicated Patient Care Coordinator to join our nursing staff. This role offers the opportunity to provide exceptional patient care and support in a fast-paced hospital environment.
Job Summary
We are looking for an individual who is passionate about delivering high-quality patient care and is committed to providing excellent service to our patients, families, and visitors. The successful candidate will be responsible for assisting with patient care, transportation, and documentation, as well as providing clerical support to the nursing team.
Responsibilities
- Assist with patient care under the supervision of licensed personnel
- Transport patients via wheelchair or stretcher
- Provide observation services for patients requiring constant supervision
- Perform general receptionist and clerical duties to support the nursing unit
Qualifications
- High school graduate or equivalent
- Previous hospital or nursing home experience preferred
- Current RI Nursing Assistant License and Basic Life Support certification required
Salary Information
This position offers a competitive salary range of $35,000 - $45,000 per year, depending on experience.
