We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality care to patients in their homes. As a PRN Home Care LPN, you will have the opportunity to work with a variety of patients and families, and make a meaningful difference in their lives.
Responsibilities:
* Provide and direct provisions of nursing and personal care to patients in their homes as prescribed by the physician or as authorized by the state/program and in compliance with applicable laws, regulations, and agency policies.
* Coordinate the plan of care with other health care professionals involved in care and help to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, case manager and other community resources.
* Provides high quality clinical services within the scope of practice and within infection control standards, in accordance with the plan of care/service plan, and in coordination with other members of the patient/client's care team from admit through discharge.
Requirements:
* Current LPN licensure in the state of Florida
* Ability to work independently and as part of a team
* Strong communication and interpersonal skills
* Ability to provide high-quality care to patients in their homes
