Oak Park Oasis is seeking a full-time Psychiatric Rehabilitation Service Coordinator (PRSC) to provide coordination of services, programs, and oversight of data collection for residents. The ideal candidate will have a strong background in human services and experience working with the adult mentally ill population.

Key Responsibilities:

  • Coordinate services and programs for residents, including crisis intervention and documentation.
  • Review resident performance and provide feedback to interdisciplinary teams.
  • Develop and implement care plans, involving families in the process.
  • Manage caseloads, facilitate skills groups, and provide psycho-social support.
  • Collaborate with interdisciplinary teams to ensure seamless care delivery.

Requirements:

  • Minimum of one year long-term nursing care experience working with the adult mentally ill population.
  • BA Degree in Human Services field (including sociology, social work, special education, and psychology).
  • Excellent assessment, interpersonal, and communication skills.
  • Knowledge of MDS 3.0 and basic computer skills.
  • Ability to work in an interdisciplinary team environment and handle confidential data with professional discretion.
