Pediatric Medical Social Worker Job Description

At ChildServe, we are seeking a skilled Pediatric Medical Social Worker to join our team. As a key member of our interdisciplinary team, you will provide social services to children, young adults, and their families within our outpatient clinics.

Key Responsibilities:
  • Communicate with families and referrals as the initial point of contact for outpatient services
  • Advocate for children and families in accessing needed benefits and services
  • Collaborate with the interdisciplinary team to develop treatment plans and ensure the needs of the child and family are met
  • Assess strengths and emotional needs of families and children with special healthcare needs
  • Identify barriers for child and family participation in treatment or achievement of treatment goals
  • Serve as the primary liaison for the Department of Human Services
  • Facilitate discharge-planning process by making necessary discharge and transfer referrals in a timely fashion
  • Maintain current knowledge of community and ChildServe resources
  • Educate and inform children and families regarding child/family rights and consent for services
Requirements:
  • Master's degree in social work from an accredited School of Social Work
  • Licensed to practice social work in Iowa, with an active license without disciplinary action
  • Preferred: Three years of social work practice in a medical setting
  • Preferred: Experience practicing as a Medical Social Worker in a pediatric setting
  • Ability to communicate in English effectively for understanding, in writing and verbally
