Nurse - Resident Care Specialist

As a key member of our care team, you will be responsible for providing high-quality care to our residents, ensuring their physical, emotional, and social needs are met.
Responsibilities:- Conduct regular wellness visits to assess residents' health and identify potential issues.
- Develop and implement individualized care plans to meet residents' unique needs.
- Collaborate with the Resident Care Director to ensure seamless care delivery and effective communication with residents and their families.
- Monitor and report any changes in residents' health status, including falls, accidents, or incidents.
- Document all pertinent information in residents' wellness files, ensuring accurate and timely updates.
- Communicate effectively with residents, families, and healthcare providers to ensure comprehensive care.
- Participate in quality improvement initiatives to enhance care delivery and resident satisfaction.
- Graduate of an approved college or school of nursing.
- Current state license as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
- Minimum of one year of experience in a healthcare setting, preferably in senior care.
- Strong knowledge of nursing practices, techniques, and methods applied to health and wellness resident care.
- Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare providers.
