We are seeking a highly skilled Patient Care Coordinator - Registered Nurse to join our team at Siouxland Community Health Center. As a key member of our healthcare team, you will play a vital role in ensuring the delivery of high-quality patient care.
Key Responsibilities- Triage and Communication: Respond to patient phone calls, triage their needs, and document interactions accordingly.
- Scheduling and Referrals: Schedule appointments, make referrals, and coordinate patient care with healthcare providers.
- Electronic Medical Records: Access, update, and maintain accurate patient information in our electronic medical record system.
- Medication Management: Refill patient medications according to SCHC protocol and policy.
- Documentation and Education: Prepare, update, and document patient records, as well as educate patients on health information and distribute relevant materials.
- Follow-up and Coordination: Ensure proper follow-up care for patients discharged from emergency departments or hospitals, and coordinate with healthcare providers as needed.
- Quality Improvement: Participate in process improvement initiatives, provide feedback, and suggest improvements to enhance departmental workflow and patient care.
- Current RN licensure
- Current BCLS and Mandatory Reporter certification
- Experience with electronic medical records (EMR) and computer skills
- Ability to work in a fast-paced environment and prioritize tasks effectively
