Responsibilities- Gather and verify patient information for accurate Home Health and Hospice admissions
- Utilize strong communication skills to work with individuals and their families
- Maintain accurate records and reports to ensure compliance and efficient operations
- 3-5 years of healthcare experience, preferably in home care
- Exceptional organization and time management skills
- Excellent communication and interpersonal skills
- Able to work independently and as part of a team
The estimated annual salary for this role is $63,000 - $73,000, depending on qualifications and experience.
