About the Role

The work you do here has a direct impact on the health and well-being of our members. As a Field-Based Community Health Worker (CHW), you will act as a liaison between Medicaid members and healthcare providers. Your primary responsibilities will include assessing member needs, developing individualized care plans, and providing community resources and benefit education.

Schedule and Work Environment

This is a field-based role with a home-based office, requiring local travel up to 75% of the time. You will spend most of your time engaging directly with members, either face-to-face or telephonically. The schedule is Monday through Friday, standard business hours, with no nights, weekends, or holidays.

About the Ideal Candidate

We are looking for a motivated and compassionate individual with experience assisting others with healthcare and social determinants of health needs. A high school diploma/GED and intermediate computer proficiency are required. Familiarity with community resources and the ability to travel up to 75% of the time are also essential. If you reside within Salina, KS, you may have the flexibility to work remotely and in the field.

Salary and Benefits

The estimated salary for this position is $40,000-$60,000 per year, depending on location and experience. UnitedHealthcare offers a comprehensive benefits package, including medical, dental, and vision coverage, as well as retirement savings options and paid time off.
