This is a full-time position (40 hours per week). The health center operates two evenings until 8:00 PM and Saturdays from 8:00 AM to Noon. The schedule may include evening and Saturday rotations.

Role:


The Registered Nurse delivers nursing services at the highest level of the incumbent's license to patients, adhering to all applicable Massachusetts State Laws and regulations set forth by the Department of Public Health and Harbor Health Services, Inc.


Key Responsibilities:

  • Administers routine nursing care to patients, following age-specific guidelines and standard precaution practices at all times.
  • Collaborates with the care team and/or provider to develop and implement a comprehensive care plan.
  • Administers medications and vaccinations to patients via various routes as prescribed by the provider or according to standing orders.
  • Ensures that emergency equipment in the clinical area is properly maintained and functional.
  • Participates in team, department, and clinic meetings to enhance learning, support process improvements, and contribute to care plans as appropriate.
  • Provides cross-coverage and training for staff in similar roles as needed.
  • Offers health coaching and education for common conditions (e.g., asthma, diabetes) through individual nurse visits, group visits, and co-visits with other disciplines as necessary.
  • Assists patients and families in staying current with clinical care guidelines, immunizations, and health maintenance tests.
  • Facilitates transitions of care through nurse visits following emergency department or hospitalizations as appropriate.

Preferred/Required Qualifications:

  • Current Massachusetts Registered Nurse (RN) license and Basic Life Support (BLS) certification required; BSN preferred.
  • Strong customer service and communication skills, both verbal and written.
    Bilingual candidates (Vietnamese, Spanish, Portuguese, or other languages) are encouraged to apply.
  • Ability to work effectively in both team settings and independently.
  • Experience working with diverse populations, with bilingual skills being a plus.
  • Basic computer skills, including proficiency in Microsoft Office (Word, Excel, Outlook) and familiarity with Electronic Medical Record software, particularly EPIC/OCHIN, is highly desired.
  • Experience in community health settings, ambulatory care, or health insurance environments is preferred.
