About the Role

The Behavioral Health Care Coordinator role is an essential element of our Integrated Care Model. As a key member of our team, you will assess, plan, and implement care strategies that meet the unique needs of each patient. Your expertise in behavioral health and case management will enable you to identify and initiate referrals for social service programs, ensuring patients receive the support they need to thrive.

Responsibilities
  • Assess and develop individualized care plans that address patients' complex medical and behavioral needs
  • Coordinate with healthcare professionals, community resources, and family members to ensure seamless care delivery
  • Visit Medicaid members in their homes, community centers, hospitals, or providers' offices to provide personalized support and advocacy
Requirements
  • RN license in Texas or equivalent credentials (LPC, LBSW, LMSW)
  • 2+ years of experience in behavioral health settings
  • Intermediate proficiency in MS Word, Excel, and Microsoft Outlook
  • Reliable transportation and ability to travel up to 75% of the time within the assigned region
