The Providence Transitional Care Center features 52 beds, offering a balanced mix of acute care residents and long-term care patients. This unit is designed for stability, providing a dynamic and engaging work environment with 12-18 Medicare beds.

Essential Qualifications:

Providence Home and Community Care (HCC) is a service line within Providence, comprising over 8,000 caregivers and 2,000 volunteers who serve nearly 200,000 individuals annually in various settings, including homes, clinics, and community living facilities across our extensive service area.
