Job Purpose:
Enhance the health outcomes of the communities we serve by assisting patients with complex chronic illnesses through the following responsibilities:
Key Responsibilities:
- Evaluate registries and reports to identify patients requiring additional support for complex chronic conditions, facilitating the development of tailored care plans.
- Conduct comprehensive patient chart reviews and interviews to uncover clinical and social determinants affecting health.
- Collaborate with patients, CHAS Health care teams, and external organizations to ensure coordinated care.
- Assist patient care teams in formulating care plans and implementing them to promote patient engagement and adherence to treatment protocols. Provide patient education as necessary.
- Work alongside case management and other resources to address social determinants of health challenges.
- Oversee effective transitions of care for patients leaving hospital environments.
- Help track and report outcomes for designated populations to leadership.
- Perform additional duties as assigned, supporting the mission and core values of CHAS Health.
Education: A Bachelor's degree in nursing from an accredited institution or equivalent experience is required. A Registered Nurse (RN) license in the State of Washington is mandatory upon hire, with eligibility to obtain an RN license in Idaho. Experience in community health settings is preferred. Familiarity with Chronic Disease Management and electronic health records is advantageous.
Skills: Strong customer service, oral and written communication skills are essential. A compassionate approach to patient care is required. Ability to thrive in a team-oriented environment and manage multiple tasks in a dynamic setting is necessary. Commitment to fostering a safe, respectful, equitable, and inclusive workplace is essential. Proficiency in computer skills is required. CPR/AED certification for Health Care Providers is mandatory. Ability to mentor students as needed. A valid driver's license and insurance are required.
Physical Requirements:
While performing the duties of this role, the employee is expected to stand, sit, and move frequently throughout the day. Manual dexterity is required for handling tasks over two-thirds of the time, along with reaching with hands and arms. Climbing or balancing is infrequent, while stooping, kneeling, or crouching occurs regularly. Effective communication through speaking and listening is necessary for the majority of the day. The role may involve lifting or exerting force up to 10 pounds frequently and occasionally up to 25 pounds. Rarely will lifting exceed 26 pounds.
