Job Summary

The RN Case Manager is a critical role at Princeton Baptist Medical Center, responsible for coordinating patient care across the continuum to ensure optimal health outcomes and efficient resource utilization.

Key Responsibilities
  • Conduct thorough medical necessity screenings and submit for Physician Advisor review
  • Coordinate patient care through effective communication with healthcare teams
  • Develop and implement transition plans to promote timely throughput and safe discharge
  • Collaborate with physicians, office staff, and ancillary departments to ensure seamless care coordination
  • Lead and facilitate multi-disciplinary patient care conferences to drive quality care
  • Manage concurrent disputes and make referrals to other departments as needed
  • Identify and refer complex patients to Social Work Services for additional support
  • Communicate effectively with patients and families about the plan of care
  • Document patient information accurately and timely in the Case Management system
Requirements
  • Graduate of an accredited school of nursing
  • 2 years of acute hospital patient care experience
  • Active RN license for the state(s) covered
  • Preferred: Academic degree in nursing, acute hospital case management experience, and Accredited Case Manager (ACM) certification
Physical Demands

The RN Case Manager will be required to lift up to 25 lbs, push/pull up to 25 lbs of force, and perform frequent sitting, moderate standing, walking, reaching, stooping, and bending.
