Job Title: Registered Nurse Hackensack NJ

Job Summary:

Vitality Home Healthcare is seeking a skilled Registered Nurse to join our team. As a Home Health Registered Nurse, you will play a critical role in providing high-quality, compassionate care to patients in the comfort of their own homes.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' health status and individual needs.
  • Develop personalized care plans in collaboration with patients, their families, and other healthcare professionals.
  • Provide skilled nursing care, including wound care, medication management, and monitoring vital signs.
  • Administer treatments and medical procedures as prescribed by physicians.
  • Educate patients and caregivers on disease management, medication administration, and lifestyle modifications.
  • Coordinate with other members of the healthcare team to ensure continuity of care and effective communication.
  • Document patient care activities accurately and timely in electronic health records.
  • Monitor and evaluate patients' progress, adjusting care plans as needed to achieve optimal outcomes.
  • Provide emotional support and advocacy for patients and their families.
  • Adhere to all regulatory guidelines, including Medicare regulations, as well as agency policies and procedures.

Qualifications:

  • Current Registered Nurse (RN) license in the state of NJ.
  • Bachelor's degree in nursing (BSN) preferred.
  • Minimum of two years of clinical experience, preferably in home health or a related setting.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficiency in using electronic health records and other healthcare technology.
  • Compassionate and patient-centered approach to care.
  • Reliable transportation and valid driver's license.
  • CPR certification required.

Physical Requirements:

  • Ability to lift and move patients safely, as needed.
  • Comfortable working in various home environments, including stairs and uneven surfaces.
  • Ability to withstand long periods of standing, bending, and walking.
  • Good manual dexterity and hand-eye coordination for performing nursing procedures.
