Key Responsibilities- Oversee and direct the activities of the case management department, ensuring seamless coordination of patient care and services.
- Facilitate the hospital's quarterly Utilization Management Committee, ensuring compliance with CMS' Conditions of Participation.
- Lead and coordinate departmental-specific performance improvement initiatives, reporting to the hospital's Quality Improvement Committee.
- Collaborate with interdisciplinary teams to identify barriers to care and develop solutions, ensuring timely and effective patient discharge.
- Ensure accurate and thorough case management documentation, adhering to workflow timeline requirements and CMS guidelines.
- Schedule family conferences and communicate with caregivers to keep patients and designated caregivers informed of progress and plans for discharge.
- Coordinate weekly patient care team conferences to develop, monitor, and refine treatment plans, achieving identified patient goals and outcomes.
- Review patient assigned CMG and identify potential missed comorbid conditions, communicating findings to the HIM team.
- Current Registered Nurse or Social Work licensure or Masters or Bachelors in Social Work as required in the state where the hospital is located.
- Certification in Case Management or Rehabilitation preferred.
- Minimum of 2 years social work or case management experience in an inpatient setting, with acute/rehabilitation hospital experience preferred.
- Previous leadership or supervisory experience strongly preferred.
- Effective oral and written communication skills in English, with additional languages preferred.
