Primary Care Physician Leader

**Job Summary**
We are seeking a highly motivated and compassionate Primary Care Physician to join our team at One Medical Seniors in Phoenix, AZ. As a Primary Care Physician, you will be responsible for delivering exceptional care to our geriatric patient population and working closely with the Medical Directors to develop and implement comprehensive care plans.
The ideal candidate will have a strong background in primary care, with a focus on gerontology and chronic disease management. You will have the opportunity to work in a collaborative environment with a talented team of healthcare professionals who share your commitment to excellence.
**Responsibilities**
- Provide high-quality, patient-centered care in an outpatient setting
- Develop and implement comprehensive care plans that address the unique needs of our patients
- Collaborate with the care team to leverage coaching and integrated behavioral health models
**Requirements**
- At least 2 years of primary care experience required
- Board Certification in Internal or Family Medicine required
- Ability to build successful relationships with team members and communicate effectively
