Key Responsibilities- Collaborate with patients, caregivers, nurses, social workers, physicians, payers, and support staff to develop and implement comprehensive care plans.
- Conduct thorough assessments of patients' needs and develop strategies to address social determinants of health and care complexities.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure high-quality patient care.
- Participate in quality improvement initiatives to enhance the care management process and patient experience.
- Bachelor's Degree in Nursing (BSN) or higher
- Current California Registered Nurse License
- 2 years of experience in healthcare, including 2 years in case management or care coordination
- Successful completion of the Transitions in Practice (TIP) program for Care Manager or equivalent experience
- Master's Degree in Healthcare-related field
