We are seeking a skilled Registered Nurse to join our team as a Case Manager. In this role, you will be responsible for conducting intake assessments with patients and families, ensuring they feel equipped and supported. You will develop individualized treatment plans that account for physical, financial, and other constraints. Additionally, you will adjust treatment protocols as needed, suggest and coordinate follow-up services to promote wellness, and ensure patients have transportation to and from their homes and healthcare facilities. You will also meet with healthcare teams to discuss patients' progress and deliberate on appropriate care. Furthermore, you will check if patients have sufficient insurance coverage or finances to pay for their treatment and devise innovative solutions or suggest referrals for patients who cannot afford treatment.
Key Responsibilities:
• Conduct intake assessments with patients and families to understand their needs and develop individualized treatment plans. • Ensure patients feel equipped and supported throughout their treatment journey. • Develop and adjust treatment protocols as needed to ensure optimal patient outcomes. • Suggest and coordinate follow-up services to promote patients' wellness. • Ensure patients have transportation to and from their homes and healthcare facilities. • Meet with healthcare teams to discuss patients' progress and deliberate on appropriate care. • Check if patients have sufficient insurance coverage or finances to pay for their treatment. • Devise innovative solutions or suggest referrals for patients who cannot afford treatment.
Requirements:
• Bachelor's degree in nursing. • Advanced degree in nursing is strongly advantageous. • Certificate in community and/or hospital case management. • Prior experience as an RN case manager within a similar healthcare facility. • Driver's license and own vehicle. • Familiarity with patient databases. • Strong understanding of ethics surrounding the storage of patient information. • Exceptional leadership, supervision, and motivational skills. • Committed to promoting patients' well-being.
