School Case Nurse

Aveanna Healthcare LLC is seeking a skilled LPN to provide private duty nursing services in a school setting. As a School Case Nurse, you will work with medically fragile patients to help them reach their full potential and improve their health conditions.
Responsibilities- Implement physician-prescribed plans of care
- Conduct patient assessments and coordinate care
- Administer prescribed medication, treatments, and therapies
- Document care provided in a timely and compliant manner
- Teach and train family members on health promotion and disease management
- Accept and write verbal orders from physicians as required
- Follow up with physicians, clients, and family members on client problems and concerns
- Active, unencumbered LPN license in the state of practice
- CPR certification from American Red Cross or American Heart Association
- TB skin test within the last 12 months
- Six months prior hands-on nursing experience preferred
- Reliable transportation
