About the Role

VNS Health is seeking a skilled Registered Nurse to fill the position of Field Supervisor in our Home Health department.

Key Responsibilities
  • Provide clinical leadership and support to Home Health Aides in the field, ensuring high-quality patient care and adherence to care plans.
  • Conduct initial and ongoing supervisory visits to patients' homes to assess their needs, provide training, and coordinate care.
  • Document patient visits and care plans in the Electronic Medical Record (EMR) in a timely and accurate manner.
  • Orient and supervise Home Health Aides in the field, ensuring they have the necessary skills and knowledge to provide excellent patient care.
  • Collaborate with the clinical care team to convey significant changes in patient condition, emergency interventions, or care plan changes.
Requirements
  • Current license to practice as a Registered Nurse in New York State.
  • Minimum of two years of experience as an RN in a home care setting.
  • Valid driver's license may be required.
