Key Responsibilities- Provide program orientation to patients, families, and caregivers, including an overview of the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of the patient's treatment plan, supporting their strengths, abilities, needs, and preferences.
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences to the patient, family, and caregiver.
- Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
- Associate Degree in Nursing or Nursing Diploma, Required
- Bachelor's Degree in Nursing, Preferred
- Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation
- 2+ years experience in case management OR 3+ years experience in clinical nursing, Required
- InterQual experience, Preferred
- Certification in Case Management, Preferred
