We are seeking a skilled Registered Nurse to join our Care Management team as a Care Manager. As a key member of our integrated Care Team, you will work closely with patients, their medical providers, and other healthcare professionals to develop and implement personalized care plans.
Key Responsibilities- Work with patients to identify and address their unique needs and goals
- Develop and implement care plans that promote optimal health outcomes and quality of life
- Collaborate with medical providers to ensure seamless transitions of care
- Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle choices
- Monitor patient progress and make adjustments to care plans as needed
- Graduate of an accredited School of Nursing
- Currently licensed as a Registered Nurse in the State of the posted location
- 2+ years previous experience working in care management and/or with CKD/ESRD patients
- Excellent verbal communication skills and ability to build strong relationships with patients and healthcare professionals
