Job Summary

We are seeking a skilled Licensed Vocational Nurse to join our team at Harbor Haven Health & Rehabilitation in Fond du Lac County, WI. The successful candidate will provide high-quality nursing care to residents on an assigned unit, working under the general direction of the Director of Nursing.

Key Responsibilities
  • Direct the planning of individual nursing care plans, ensuring the delivery of high-quality care to residents.
  • Communicate effectively with other disciplines involved in patient care, including physicians, therapists, and other healthcare professionals.
  • Supervise nursing procedures and performance, investigating personnel problems and completing staff performance appraisals.
  • Ensure adequate staff coverage for units, reviewing and revising nightly care sheets and responsibility lists.
  • Direct the operation of the units, maintaining and ordering unit supplies, and completing inventory checks.
  • Ensure a safe, therapeutic environment that protects patients' rights and safety for residents and staff.
Requirements
  • College degree in nursing.
  • One year of medical/surgical or geriatric nursing experience.
  • Wisconsin Registered Nurse license.
  • Knowledge of geriatric, DD, and CMI needs and concerns, federal and state regulations, current nursing practice, and residents' rights.
  • Ability to make accurate assessments, provide crisis intervention, communicate effectively orally and in writing, and interact effectively with patients, their families, and the general public.
