Key Responsibilities
  • Develop and implement individualized service plans to meet the unique needs of clients
  • Provide case management services, including assessment, planning, and coordination of care
  • Collaborate with clients, family members, and other healthcare professionals to ensure seamless care coordination
  • Maintain accurate and up-to-date records of client interactions and service delivery
  • Meet performance targets and unit measures to ensure high-quality service delivery
Requirements
  • Bachelor's degree in a related field, such as psychology, social work, or counseling
  • At least 1 year of experience in the mental health or related field
  • Valid Texas Driver's license and auto liability insurance
  • Ability to pass criminal history and background checks, as well as a pre-employment drug screen
  • QMHP certification within 6 months
