Atrium Health is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our home healthcare team in University City. As an LPN, you will play a vital role in delivering high-quality patient care and making a meaningful difference in the lives of our patients.

Job Summary:
This position involves performing nursing care and assisting with planning and implementing care for the comfort and well-being of patients. You will work under the supervision of a Registered Nurse (RN), physician, or other licensed independent practitioner, ensuring that tasks are completed efficiently and effectively.

Essential Functions:
* Assessment: Participate in determining nursing care needs based on collection and interpretation of relevant data related to patient health status.
* Recognize and respond to immediate interventions as needed.
* Demonstrate knowledge enabling documentation of patient observations, treatments, patient response to treatment, and nursing care utilizing the nursing process; actively participate in treatment and assist with procedures; and actively participate in discharge planning.
* Assist RN in developing/revising the plan of care by providing data and identifying potential goals and intervention for review by the RN.
* Communicate with clinical staff on patient status if a significant change has occurred.
* Implement identified plans under the supervision of the RN, physician, or other independent licensed practitioner, ensuring tasks have been completed.
* Aware of patient's progress and goals, as demonstrated by documentation in medical records and reports same to the Registered Nurse.
* Organize workload and prioritize responsibilities to ensure quality patient care delivery in a time-efficient manner.
* Attain knowledge and competence reflecting current nursing practice and perform only those nursing skills which they are competent to perform in accordance with legal boundaries and facility policies and procedures.
* Document assessments, procedures, patient education, and communication of information to appropriate members of the healthcare team in a timely manner.

Physical Requirements:
The job requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling. Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies. Must speak English in good, understandable terms. Intact sense of sight, hearing, smell, and touch. Finger dexterity. Critical thinking and ability to concentrate. Must be able to respond quickly to changes in patient conditions. Physical Abilities Testing required. Additional department-specific physical requirements may be identified for unique responsibilities within the department by the nurse leader.

Education, Experience, and Certifications:
* Graduate from an accredited School of Practical Nursing. BLS required per policy guidelines. Current LPN license or temporary license as a Licensed Practical Nurse Petitioner in the state where you work and reside, or meet the licensure requirements in your home state. Additional education, training, certifications, or experience may be required within the department by the nurse leader.
