Key Responsibilities
  • Provide skilled nursing interventions and professional nursing services to patients in their homes
  • Maintain effective communication with patients, families, and healthcare team members to enhance care coordination and achieve optimal patient outcomes
  • Utilize a holistic approach to patient care, including treatment, rehabilitative needs, and preventive measures
Requirements
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
  • Current nursing license in the practicing state
  • Valid drivers license, auto insurance, and reliable transportation
  • Current CPR certification
  • Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting
Compensation and Benefits

The estimated salary for this role is $45,000 - $62,500 per year, depending on experience and qualifications. In addition to competitive pay, we offer a range of benefits, including paid time off, 401(k) retirement savings plan, employee assistance program, business travel, and accident insurance.
