Role Summary

We are seeking a motivated individual to join our team as a CARE COORDINATOR. This role is essential in managing care for our program participants, ensuring they receive the support they need throughout their recovery journey.

Key Responsibilities:

1. Develop and implement individualized care plans based on patient assessments.
2. Facilitate life skills workshops in accordance with organizational policies.
3. Serve as a liaison to establish partnerships with hospitals, community health organizations, and other service agencies to address patient needs.
4. Maintain accurate documentation of admissions, care management, and discharge processes.
5. Conduct comprehensive case management assessments to identify patient requirements and ensure completion of necessary evaluations.
6. Coordinate patient schedules, including bed assignments and transitions in care levels, while maintaining communication with relevant coordinators.
7. Assist the treatment team with patient intake, transfers, and discharge evaluations.
8. Collaborate with multidisciplinary teams to identify and address patient needs effectively.
9. Support patients in accessing essential resources, including medical, legal, and spiritual assistance.

Qualifications

Education and Experience:
Preferred candidates will possess an associate's or bachelor's degree in human services or a related field. Additionally, the ability to obtain a Certified Addiction Technician (CAT) certification within 18 months is required.

Other Requirements:
1. Strong understanding of substance use and mental health treatment systems.
2. Proficient computer skills are necessary.
3. CPR and first-aid certification (training provided).
4. Capability to monitor vital signs and identify potential medical issues.
5. Valid driver's license with a clean driving record.
6. Effective communication skills within a multidisciplinary treatment framework.
