Optum Care Advanced Clinician Position

About the Job
In this role, you will provide comprehensive care to patients in their homes and virtually, including diagnosing and treating acute and chronic conditions, ordering diagnostic tests, prescribing medications, and performing procedures as needed. Your ability to work independently and collaboratively with the interdisciplinary team will be essential in this role. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for delivering compassionate care to older adults.
Key Responsibilities
- Develop and implement individualized care plans for patients
- Conduct thorough physical and psychosocial assessments of patients
- Order and interpret diagnostic lab work, x-rays, and EKGs
- Prescribe pharmacological and non-pharmacological interventions for acute and chronic conditions
- Perform procedures based on patient condition, such as placing IVs, wound debridement, joint injections, vaccinations, etc.
Qualifications
- Master's degree in Nursing or Physician Assistant Studies from an accredited program
- Current licensure through the State Board of Nurse Examiners or State Medical Board
- Current prescriptive authority from the State Board of Nursing or State Medical Board
- Comfortable with chronic condition management for Medicare-aged population
- Must have a valid driver's license within the state of work
- Ability to establish effective working relations with patients and their families
