- Lead and manage a team of healthcare professionals, providing guidance, support, and development opportunities to enhance their skills and performance.
- Collaborate with interdisciplinary teams to ensure comprehensive care planning and coordination.
- Monitor and evaluate clinical outcomes, identifying areas for improvement and implementing strategies to enhance patient care and satisfaction.
- Ensure compliance with corporate standards, regulatory requirements, and industry best practices.
- Associate degree in science or a bachelor's degree from a School of Nursing is preferred.
- 5 years of experience as a registered nurse in a Home Health service or Hospice environment, with a minimum of 2 years in a leadership role.
- Current State License as a Registered Nurse.
- Valid driver's license and insurance, with reliable transportation to perform job tasks.
- 10% travel required.
